Specimen Requirements
- Prefixation with alcohol is not recommended for spinal fluid, because alcohol may precipitate protein present in the fluid specimen.
- Submit 5 mL of fresh spinal fluid in a screw-capped, sterile container.
- If specimen cannot be sent to laboratory immediately, submit 5 mL of fresh spinal fluid in a screw-capped, sterile container to which equal amounts of 50% ethanol alcohol has been added.
- If spinal fluid is not fixed with preservative, immediately deliver to Cytology Laboratory.
- Label container with patient’s first and last name and second identifier, date of collection, physician’s name, and hospital identification number (if applicable)
- Please complete a Cytology Request Form including patient’s name, date of birth, date of collection, physician’s name, address, specimen source, and pertinent clinical history; and forward it with the specimen.
- Note on Cytology Request Form whether tumor cells or leukemia cells are being ordered. Specimen cannot be processed unless this is known.
- Place specimen in a plastic specimen bag with Cytology Request Form inserted into pocket separate from specimen; and, if possible, deliver specimen to Cytology Laboratory.
Specimen Rejection
The following specimens will be returned to submitting physician:
- Spilled specimens
- No requisition form
- Name on requisition does not match name on specimen
- Unlabeled specimen
- No doctor’s name given

Interpretation of cells found in spinal fluid is difficult due to the small volume of specimen normally received, the paucity and distortion of cells, and commonly extensive cellular degeneration resulting from the lapse between time of cellular exfoliation and collection of the specimen. The presence of contaminants from any source may be disastrous to interpretation and should be avoided both during specimen collection and specimen processing.
Methodology
Includes testing for leukemia or malignant cells.
Cytologic evaluation of spinal fluid is useful in diagnosis of certain primary and/or metastatic cerebral tumors as well as follow-up of patients with confirmed lymphoma or leukemia. It may also be helpful in determination of the nature of certain cerebral infections such as cryptococcal meningitis.

Specimens received after hours, weekends, or holidays will be processed the next business day.
